New L&GNSW Direct Debit Form for Club Taxes and CMS Fees

• Revenue NSW – for Gaming Machine Taxes
• Data Monitoring System (a division of Maxgaming) – for CMS monitoring fees.

New Form 

Clubs are required to provide the direct debit to L&GNSW when applying for or transferring a licence. The below direct debit request form should be used, which includes the current Customer Service Agreement and authorisation.
